Community Living, Independent Life
For some individuals that Damar serves, living independent lives in the community presents special challenges. With the appropriate level of support, they can. Damar provides that support through its Community Living & Support Services program, also known as “CLASS.”
With CLASS, Damar helps adults find the right living situations and take on appropriate responsibilities. Through this process, it helps families, caregivers, and the individuals themselves identify the right home setting – whether that’s alone, with family, or with roommates – and then assists the individual as needed with daily tasks and responsibilities.
Once a client is in his or her home, Damar continues to help by providing daily assistance, life-skills training, employment support, and other services – all with the aim of helping the individual realize their potential, fulfill their dreams, and live increasingly meaningful lives.
Damar’s services are provided in a number of ways:
These services, provided around the clock, are primarily funded through Indiana’s Community Integration & Habilitation Waiver and Family Support Waiver. Applications and eligibility determination for Medicaid Waiver services can be obtained from the Bureau of Developmental Disabilities Services.
These services are provided only as much as is necessary to ensure the individual is safe, secure, and living successfully. They are funded primarily by Indiana’s Community Integration & Habilitation Waiver and Family Support Waiver, as well as CHOICE. Applications and eligibility determination for Medicaid Waiver services can be obtained from the Bureau of Developmental Disabilities Services. To learn about applications and eligibility for CHOICE, contact the Area Agencies on Aging.
These services help groups of participants obtain, keep, and strengthen the skills they need to participate in the community and function independently outside the home. This could include activities such as accompanying participants to the grocery store or restaurants, teaching them how to enjoy appropriate social and recreational options, and assisting them in other activities of community life.
Through these services, Damar works one-on-one with each participant to help him or her develop the skills needed to become a part of a community. Participants learn about appropriate social, recreational and leisure activities as they receive guidance on matters such as accessing the public library, using a local community recreation center, and more.
