	Community Living & Support Services Damar’s Community Living & Support Services program offers less than 24-hour in-home support to children and youth. This program is funded primarily by the following home and community-based Medicaid waivers in Indiana: Autism Developmental Disabilities Support Services Aged & Disabled Traumatic Brain Injury CHOICE Applications and eligibility determination for Medicaid Waiver services can be obtained from the Bureau of Developmental Disabilities Services. Applications and eligibility determination for CHOICE can be obtained by the Area Agencies on Aging.
