Community Living & Support Services

Damar’s Community Living & Support Services program offers less than 24-hour in-home support to children and youth. This program is funded primarily by the following home and community-based Medicaid waivers in Indiana:

• Autism
• Developmental Disabilities
• Support Services
• Aged & Disabled Traumatic Brain Injury
• CHOICE

Applications and eligibility determination for Medicaid Waiver services can be obtained from the Bureau of Developmental Disabilities Services.

Applications and eligibility determination for CHOICE can be obtained by the Area Agencies on Aging.

Community Habilitation Services
Community Habilitation offers stimulating activities throughout the day for people with developmental disabilities as well as offering volunteer opportunities and access to community supports. Community Habilitation is offered in an individual or a group setting.

Community Habilitation-Group
Community Habilitation-Group services are designed to assist the client in acquiring, retaining, or improving self-help, socialization, and adaptive skills required for active community participation and independent functioning outside the client's home or family home. Activities may include accompanying individuals to the grocery store, or eating establishments; teaching an individual how to participate in appropriate social and recreational activities; and assessing other activities of community living. These services are provided in a group setting.

Community Habilitation - Individual Community Habilitation-Individual services are designed to provide one-on-one services to the client. This involves one staff member assisting one client in developing community integration skills. Participants will realize the benefits of community involvement as they are more fully aware of activities such as social, recreational, and leisure activities. Examples of this service may include assisting the individual in accessing the public library or local community recreation center.
